Introductory Consultation
Please contact me to schedule your 15 minute consultation. During the call, I will ask about what you are struggling with and briefly share how I think I can help. From there, we will schedule your first session. If it does not seem like the right fit for therapy, I am happy to offer other referrals.
Building & Parking
My office is located at 1030 Sir Francis Drake Blvd. in Kentfield, right across the street from College of Marin. There is parking onsite; during afternoon hours, please plan to arrive a few minutes early in case parking is limited. I will not be able to accommodate extra time if you are late.
First Session
I generally start sessions right on time and end at the 50-minute mark. You are welcome to arrive a few minutes early and sit in the waiting room inside suite 120. Please press the button on the wall next to my name, this will notify me that you have arrived. I will welcome you into the second room at the start of your session.
The first session I will gather relevant information and we will discuss your goals for therapy. Together we will create a plan based on your goals and current circumstances.
Fee Information
My fee is $225/session. I accept cash, check, or credit card. For payment by credit card, I will collect your credit card information before our first session. Your card will be charged within 24 hours after session. All other payments will be collected at the start of session. Please make checks payable to Carly Clifford Bloom, LMFT.
I do not take insurance. I am happy to provide you with a superbill that you can submit to your insurance company for possible reimbursement. Please check with your health plan for more information.
Cancellation Policy
Sessions are typically scheduled to occur one time per week at the same time and day. Consistent attendance greatly contributes to a successful outcome. Should you need to cancel your appointment, please notify me at least 24 hours in advance. If you need to cancel after 24 hours, you will be charged the full session fee. This is necessary because a time commitment is made to you and held exclusively for you.
Good Faith Estimate
You are entitled to receive a “Good Faith Estimate” of what the charges could be for psychotherapy services provided to you. While it is not possible for a psychotherapist to know, in advance, how many psychotherapy sessions may be necessary or appropriate for a given person, this form provides an estimate of the cost of services provided. Your total cost of services will depend upon the number of psychotherapy sessions you attend, your individual circumstances, and the type and amount of services that are provided to you. This estimate is not a contract and does not obligate you to obtain any services, nor does it include any services rendered to you that are not identified at the start of treatment. This “Good Faith Estimate” is not intended to serve as a recommendation for treatment or a prediction that you may need to attend a specified number of psychotherapy visits. The number of visits that are appropriate in your case, and the estimated cost for those services, depends on your needs and what you agree to in consultation with your therapist. You are entitled to disagree with any recommendations made to you concerning your treatment and you may discontinue treatment at any time. The “Good Faith Estimate” is available upon request.
